We aim to make your first visit to us as pleasant and enjoyable as possible. We immensely enjoy meeting new people and always look forward to first visits. On your first visit you will be introduced to the people who will be looking after you and who in time you will hopefully come to consider as friends and health partners.
We do value your time hence it will be great if you can print below form, fill it out and bring along with you. This will help us help you assist quick.
You will have the opportunity to ask any questions you like and will be encouraged to make yourself at home and to get to know your new surroundings. We understand that on the first visit you may like to bring a friend or relative as a familiar face and we are happy to entertain such visitors with refreshments, books and newspapers.
